How would you store this in SQL

I've taken it upon myself to parse serebii.net for my own use.
I'm working on the main pokedex and ran into something unexpected.

is almost 2 sources joined into one since its one character with 2 skill sets.

would it make sense to treat it as two separate records then join on a = character number?

(not a pokefag, just a project to keep me busy)

where I would normally put this stuff in a master table they seem like they have to be in a lookup to differentiate the two forms.

How about making separate entries for the two forms, as if it were two different pokeymanz, and giving each a "change to the other one" ability? It's hacky, but if this setup only occurs twice it might be the way to go. If there are quite a few with multiple forms, you might as well add a "forms to attacks" relation and change the "pokemon to attacks" relation into "pokemon to forms".
