Size of OS doesn't matter, Linux is written in C, which has "unsafe, undefined behavior" by default.
The larger a C program is, the probability of "unsafe, undefined behavior" increases.
If there was a type-safe, non-GC C-alternative which had the speed of C, Linux could be rewritten.

My personal opinion, is poor documentation, and poor QA testing. If you look at the FreeBSD source code, you will not see that, it will be readable, and comprehensible. And there is more than source of documentation that describe a particular section that you are looking at, that it is hard to get lost.

Imagine T.Davis was bit more sane and spend his time on improving his OS instead of posting on forums.
Now imagine T.Davis got some support. You still have a toy OS but "its something" between a full-fledged OS and pet project. Thats the stage of Minix. He has to use winXP because Minix isn't capable of much. Its programs are ancient. It has no recent drivers.
