Origin of GUI or just Output on Monitors

This has always confused the fuck out of me.

There is a question that I never understood about computer monitors? How do they know how to decode the data? Do they also have processors? Like for example, if i were to do old machine style code on the earliest monitors, how would I tell it what to do? What about in assembly? Don't these instructions only get sent to the CPU? There are so many monitors, is there a common standard?

I guess what I am getting at is how in the world are the GUI libraries of high level languages are made.

I guess another expansion of my confusion I could type.

How were the first OSes programmed? They had to be made partially in assembly, but how was all the code loaded onto these computers if they had nothing on them. There is nothing to interface with.
