So after learning HTML/JavaScript/CSS/etc. and being familiar with  how to use them, how can I learn to format pages so they actually look nice?

That requires learning some design skills and developing a sense of aesthetics.
HTML/JavaScript/CSS/etc. are just the toolbox.  The artist or craftsman is still required.

i don't know how web designers do it.
whenever i've been faced with a web project i can never be bothered making it look pretty, i just have black text on a white backround with blue hyperlinks and maybe a table or two

The Non-Designer's Design Book: Design and Typographic Principles for the Visual Novice, by Robin Williams.

I've seen this recommened from time to time, though I never bothered to actually buy it.

i don't know how web designers do it.
In Photoshop. If you don't mock up first, it's likely it will look like shit.

Also, use a restrictive CSS framework like Blueprint. It will make it harder to do horrible abominations.
