SICP chapters 4&5

So I've decided to finally read SICP and achieve satori. But looking through the contents left me with a few questions.

Chapters 4 and 5, should I read them?
What do they mean by constructing a new language? Is it simply constructing something within the program or is it REALLY making a new language i.g. python, java, ect.
Register machines, should I learn them?
I just want to be able to design my programs well and find more elegant solutions. It seems like this book is what I need.

READ SICP implies reading the entire book and not cherry-picking chapters.

Chapter five is the shit, though, seriously, it is what makes the whole book worth it if you make it to it.
