The mods should be much more strict, and just PERMANENTLY ban someone, as they'll just break the rules again, and again.
And they moderators do in many cases. And they come back, especially the spammers. Doing what you suggest will probably get rid of a few of the "casual" rule breakers who have a static IP address, but the underlying problem will still be there.
Hence why banning proxy lists would also work *Not hard to find*.
Spammers and other such abusers don't strictly rely on public Web proxy servers to do their abusive activity. Some of them may use SSH tunneling, private VPNs, private web servers that have PHP or Perl scripts that essentially turn the web server into a self made proxy, etc. The more you ban, the more you'll find that it's mostly pointless as they find new ways to get around these measures.
The only way to improve the moderation of 4chan's imageboards is more moderators. But that is much, much easier said than done. You're like the thousandth person to suggest these things and they simply do not improve the situation. 4chan is being the best 4chan it can be right now.
If it bothers you that much, I can only suggest to you that you either browse /fit/ with a text-only browser, uncheck the option in your graphical browser that says something along the lines of "load images on Web pages", or just hang out here on the textboards and maybe ask the moderators to make a textboard version of /fit/.
