Yours was a 3 day ban right? I think any ban that's more than 3 days in length includes the appeal form. (Though I have yet to confirm this. Anyone who got banned for 4 days?) As you can see in the screenshot I've written an assholish, but rightfully so appeal. It never even got reviewed, so that leads me to believe it's just an auto ban.
