Anyway, I have the hard copy of the book (you can get it off Amazon for 13 bucks) but I've been thinking of scanning it

and here it is all wrapped up as a pdf. Any mistakes are mine, it was a little bit of a rush job. Again good for 10 d/l's. Note that this is the earlier 1987 version.

I recommend finding the Language Hacking Guide by Benny for tips and inspiration.
