Xbox 360 wired controller. It has to be simply the best gamepad I've ever used.
It feels nice in the hand, has two clickable analog sticks, two analog triggers, two digital triggers, a D-Pad, four face buttons, and three buttons in the center. What more could you want?
The only issue? It's EXPENSIVE for a gamepad.

I've gotten a Mad Catz XBOX style controller which hasn't failed me yet. The triggers kind of suck but other than that, it's got a nice d-pad and the analog sticks are accurate.
Alternatively, you can get a PS -> USB converter and use an official PS1/2 Dual Shock controller. All the buttons are good the only complaint I have is that the analog sticks tend to be freaky with a retardedly small deadzone.
It's especially good because some emulators and games have a problem mapping to the POV switch which is what most d-pads are mapped to. If you disable the analog sticks by pressing that middle button, the d-pad is then mapped to the normal left right up down buttons. At least thats how it worked with the converter I had. I bought it from RadioShack for like 15 bux.

PS2->USB adapter is the best way for emulated stuff, because then you can pick any number of other controllers, and use ones you might already have/be used to.
there's a very slight input lag, but that pretty much only matters with things with down-to-the-millisecond timing like FRCs in GGXX and IIDX games. If that bothers you, use a parallel->psx/ps2 adapter and NTPad/PSXPad.
do NOT!!! get the MS sidewinder, all friend of mine and i whoa has one, and we all hate it, coz in less than a year the down button of the d-pad sands off/or grinds easily, so you will have to press REALLY hard for it to work, qcb and qcf motions becomes almost impossible :(
