Redeads and wallmasters from Ocarina of Time, the Eel from super Mario 64, That giant Dionaga from Star Wars: Shadows of the Empire, Sinistar, and The Giant Octopus from Echo the Dolphin all terrified me when I was younger. My friend's sister shrieked when she got seen by a guard in Metal Gear.

Football guy in Super Mario World.
Those guys SCARED THE SHIT out of my entire family. It was like: HOLY FUCKING SHIT ITS THE FOOTBALL GUY, FUCK THE END LEVEL JUMP JUST GET THE FUCK OUT ABORT MISSION ABORT MISSION.
All enemies scared me, because I assumed they had an actual personality as they grew up in Video Game Land and came in to my TV to try and kill my main character, who only had so many lives left before he would die, therefore it was up to me to kill them all even if it meant never getting past stage 1 and just walking back and forth.

The mind flayers and master mind flayers of NetHack are really scary :X I was up against a MMF yesterday, armed with only a cursed +0 unicorn horn, and out of tricks. Luckily the tricks I had already used managed to hurt the MMF enough that some good old bashing combined with failed brain sucking attempts managed to scare the MMF and resulted in its death.

Those flying mask things in Mario 2 freaked me out so much. When you took the key from their jars they followed you outside, which made me even more freaked out.

In Yoshi's Island, I used to be afraid of the chomps that jumped from the background and destroyed the ground. It was the noise they made. Also, I hated the touch fuzzy get dizzy level because anything fuzzy was disturbing to me.

I remember my first time playing Half-Life. I had just gotten into the sewers, and I was looking for a way to get down, and ahead of me I saw a rope. Pleased with the ease it would be to reach the bottom, I hopped onto the rope. Suddenly, the rope began to rise. I look up and scream before I am eaten by a barnacle.
You must have been screaming for a good ten minutes before the barnacle's crap damage actually killed you.

Deals like 50 damage the moment you touch the teeth. If he was low on hp he might've died immediately.

The T-Rex in Tomb Raider.
Why?
First time I faced it I accidentally fell into its pit, and it starts chasing me. I think, "No big deal, just run into that alcove over yonder and blow it away from a safe distance."
So what happens?
IT FUCKING SHRINKS AND FOLLOWS ME IN!!!
God-Damned bugs.
