List one feature, aspect, or thing that you think is bad about the design of Windows itself (rather than the performance or design of supporting applications). Also state why. Try not to list something someone else has already listed.
No OS wars, please. Do not mention any other operating systems.
I'll begin with an easy one.
Http.sys - Because kernel-mode HTTP servers are just kooky.

Windows Updater gives you updates, but requires you to restart for most of them. Also offers no way to auto-install Windows Defender updates and make the rest manual.

When you want to copy over files to a backup folder, and it asks if you want to replace the same file when it finds a duplicate, there should be a "No to all" button for that, or a "replace only if different" button.

The taskbar and buttons in Vista shouldn't have that greasy reflected effect web 2.0 bullshit look. I'd rather have plain solid colours than some half-assed special effect skinned on them.
There's a hidden secret "no to all" option, you have to hold shift when clicking [non]. Or maybe right click when clicking [no]. I don't know, I haven't used windows in ages. But you're right, the option should be clear in the dialog box.

Christ, then another criticism is the lack of written comprehensive manual to come with Windows without extra charge. My Commodore 64 came with better books. I didn't even know about Windows Scripting Host in my Windows 98SE machine until last year for Christ's sake, and I would have played around with little hobby scripts far earlier if I had any damn knowledge earlier.
