He does, but Alzheimer's is a degenerative disease, you don't get it and immediately lose your marbles. It's a long painful slide into senility. Thankfully exercising your brain regularly is thought to help stave off some of the effects, so writing will hopefully help him some.

Rincewind gets like 3 lines in the whole book, Vimes gets a single sentence, and Vetinari is even more a megaphone shitty deep-wannabe dialogue than he was in the last few books.
