The problem is quite simply this, every single thread right now are one of these:

"Post ending in ** is/gets/will be/shows __________"
"What do you think of the way I look?"
"Get on here she shows tits at 50!"
"Hey guys don't post CP" (And idiots fall for this every time, Jesus Christ)
"lol nigger racism dead cats gore scat arent i funny now guys?"
"If you've ever _______ you must post in this thread"
"Raid this website for me please"
"LET'S ALL COMPLAIN ABOUT HOW MUCH /b/ SUCKS"

The problem is that there's no way to fight it. Yes, there is a report button, but no one seems to realize that it doesn't do anything.

Threads come way too fast for it to matter, even assuming that a mod finds it and deletes a shitty thread, there are still a hundred more where that came from.
