There exist a few genetic diseases that are inherently immune to evolution; mortality associated with them occurs in the 40s, which is usually long after procreation. I can't remember the names of any such diseases, though Huntington's comes to mind, even though the wikipedia description is contrary to what I recall.

If medecine progresses fast enough, we can bypass evolution completely (genetic engineering lol). Whether or not that will happen is something I don't know.
