PHP Help

I'm trying to execute a print function with PHP when a checkbox is checked. But I'm having a hard time figuring out how to configure the checkbox to do what I want. I am also willing to do it with a drop box or what have you.

Anyone know how to do this? or a site that might be able to tell me?
