A fetish consists of a large number of memories strongly related to sexual arousal and we know that long term memory must be periodically recalled or it will be forgotten. Initially the goal is to disrupt the recall of these memories, especially during sexual activity, to stop the fetish being reinforced, which likely involves removing stimuli which triggers the fetish as much as possible. It is important to distinguish between sexual desires which are innate and the fetish which is psychological.
Sexual arousal is not like a phobia in which exposure to object that causes fear will normalize it, sexual arousal can only be relieved by sexual activity and exposure to the stimuli they find sexually arousing will only cause sexual arousal, this is why it is important for the subject to distinguish between their sexual desires and the fetish so that they can seek healthy ways of responding to their sexual desires without resorting to the fetish, thereby removing the motivation to trigger the fetish and allowing it to atrophy.
